NOTE: You will be required to submit the following documents electronically for this tournament:

Official Team Rosters (covering all Rostered, Club Pass and Guest Players)

An Event Liability Waiver (ELW), electronically signed by a Parent/Guardian for each player on your team

Scanned/Electronic copies of players cards for each participant (as provided by your state soccer governing association

Guest Player approvals (if applicable)

Out-of-State Travel Approval (if team is from outside Michigan and not a US Club member team)
