NOTE: You will be required to submit the following documents electronically for this tournament:
  • Out-of-State Travel Approval (if team is from outside Wisconsin and not a US Club team)
  • Official soccer association-approved team rosters covering all players (Rostered, Club Pass and Guest Players).
  • WYSA Event Medical Waivers for each player.
  • Guest Player Approval documentation for any Guest players used.
    • If you have been issued a SportsForms TeamID from your club, or if you have a TeamID from a previous tournament or league, you may enter it below.
